Upper Eyelid Surgery

Over time, tissues around the eye become lax due to a loss of elasticity. The redundant tissue is a sign of aging. Extra or sagging skin around the eyelids, called dermatochalasis, can be very noticeable and significantly affect one’s appearance. Upper eyelid blepharoplasty (pronounced “blef-a-ro-plasty”) removes excess eyelid skin and results in a more youthful appearance. Depending on a specific patient’s needs, the upper eyelid surgery can be customized to provide the desired cosmetic outcome. For example, one of the two muscles that open the eyelid can be tightened to achieve a better position of the eyelid margin where the lashes can be seen.

Lower Eyelid Surgery

With age, many people develop “puffy” or bulging lower eyelids. This is most commonly due to normal fat in the eye socket that has shifted forward. With age, the tissues holding the fat in place become weaker, resulting in fat protruding through the lower eyelid. In some patients, injections of filler can help improve the appearance of the lower eyelids. For others, lower blepharoplasty is a better option. The surgery is done in the outpatient setting and is customized to an individual patient’s needs and desired cosmetic outcome.

Most commonly, an incision is made on the inside surface of the lower eyelid. Through this incision, the fat is accessed and partially excised. In some patients, rather than excising the fat, it is used to fill hollow areas in the tear trough below the lower eyelid. In some patients, the lower eyelid may also need to be tightened through a small incision near the outer corner of the eyelid to provide appropriate support or improve contour. A minimally invasive mid-facelift can be performed through this same incision if needed. If there is excess skin on the lower eyelid, a small ellipse of skin beneath the eyelashes can also be excised. Dr. Riske will develop a customized approach to address your specific needs.

Brow Lift

Sagging and drooping of the eyebrows, called brow ptosis (pronounced “toe-sis”), occurs naturally with age due to weakening facial tissue in conjunction with the loss of fat. Brow ptosis can both noticeably affect one’s appearance and convey negative facial expressions of sadness or anger. There are a variety of surgical techniques used to correct brow ptosis including minimally invasive options, such as internal browpexy, a procedure that lifts the brow through an upper eyelid incision. Dr. Riske will discuss with you the option that best addresses your cosmetic concerns and facial anatomy.

BOTOX

Contraction of facial muscles can lead to noticeable wrinkles, such as “elevens” between the eyebrows and “crow’s feet” near the outer eyelids. Dr. Riske specializes in the use of BOTOX® and other injectables, such as Xeomin® and Dysport®, to decrease the appearance of such lines and wrinkles.

These injectables are helpful in targeting, amongst other areas of the face, “crow’s feet,” “elevens” between the eyebrows, horizontal lines across the forehead, and platysmal bands on the neck. Because wrinkles are caused by muscle contraction, BOTOX®, and other similar injectables work by weakening muscles and thereby decreasing wrinkles. The result is a more youthful appearance.
